Student Lab Safety Agreement
Science is a class that combines traditionally structured lessons with hands-on laboratory activities. Student will be doing a number of activities that may involve laboratory equipment, chemicals, and other items, which if used improperly, could be hazardous. Safety is the number one priority in both the science classroom and the science lab for students, teachers, and others involved. To ensure a safe environment for scientific inquiry, lists of rules have been developed in this Student Lab Safety Agreement. The student and parent must sign the agreement, but only after reading the entire contract. Students will not be allowed to participate in labs until all necessary signatures are on a turned in form. Please return within one week of receiving this agreement.
All middle school students of Cook County School District 130 know the basic rules associated with PBIS: BE RESPONSIBLE, BE RESPECTFUL, BE SAFE.
This means that we are not talking out of turn. We raise our hands to speak. We are not throwing things in the classroom. We hand items to classmates, or we place items in a garbage can. We are not getting out of our seat unexcused. We are listening to instruction and paying attention to the teacher. Please review our PBIS Matrix to understand the basic rules of the classroom and lab.
There are some additional rules that are slightly changed to fit the science classroom and laboratory setting. The following ten rules must be followed for continued participation in science activities:
1) Follow all written and verbal instructions carefully.
If you do not understand a direction or part of the procedure, ASK YOUR TEACHER BEFORE STARTING.
2) Always have teacher with you.
Do not go into the science classroom or laboratory alone.
3) Do not touch lab equipment, chemicals, or other materials unless you are instructed to do so.
Only follow the instructions given. Do not make up your own experiments or misuse lab equipment.
Remember: You break it; You buy it.
4) Do not eat food, drink, or chew gum.
5) Never fool around.
Horseplay, practical jokes, and pranks are dangerous and prohibited.
6) Be neat.
Keep work area clean, or clean up after yourself. Report any spills or glass breakage immediately
7) Stay focused.
You are working on an activity at the same time as other students. If you distract them then neither of you is doing your work to its potential. Respect yourself and respect others.
8) Know the room.
Know where fire exits, extinguishers, safety features, and the teachers are at all times in the room or lab.
9) Tie long hair back.
You do not want to ruin an experiment or your hair.
10) READ AND LISTEN CAREFULLY TO INSTRUCTIONS.
Do this before, during, and after an experiment or activity. This is very similar to rule one because it is the most important thing.
Many of these rules are simple to do if you just remember to be responsible, be respectful, and be safe.
IMPORTANT: If a student repeatedly fails to follow these rules, they will be asked to not participate in the activity, receiving a zero, and may face other disciplinary action. More often than not, this will negatively affect the student’s grade in science. Not only are labs graded, but the information discovered in doing the activities aid the student on tests and future assignments. The student will not be allowed to make the labs that they are asked to stop participation in, and the parents will be contacted when this occurs.
